WebWe are a novel service which has only been set up over the last 2-3 years and our aim is to aspire to provide hospital level care in the comfort of the patients home. We are a team that thrives to provide holistic care and are humbled by your kind and supportive words. WebThis will support reduction in hospital admission, and will expand the Dundee Enhanced Community Support Acute model into a Hospital at Home service. This pathway will be focussed on community rehabilitation in order to promote independence and has replaced the previous 28 bedded Intermediate Care Unit which was closed in March 2024.
